I ordered and used this adapter plate, it has drill marks and extra holes along the bottom edge, when I mounted to my T5 Ford Transmission.

low mileage, no warping, about 3k miles, on a low torque setup.

One ratted out hole, is not a mounting hole, doesn't affect mounting, it was simply for a manual clutch cable to pass through.

Cool yes it couples to a Ford Small Block style. I had it hooked up to a T5 transmission which is pretty common in Mustangs etc.

It does have the coupler. It is a 6 bolt pattern (not 8) so double check which flywheel you have on your Ford.
