I'm confused... I understand that there are two sizes of module in the Bolt pack, and the smaller one is shorter (in the longest dimension) but the same width and thickness, so the dimensions given make sense. But the photo appears to show six identical modules, not five long and one short.

...
Just to clarify...
That's six of the ten modules from a Bolt pack, right? That would be why it's 34 kWh (not 60 kWh), and why there are only six modules in the photo. In the Bolt, the modules lay flat, in one layer of eight large modules plus a second layer at the rear end of two small modules; the pack shown rearranges them in one stack.

This stacked pack appears to have no cooling provisions; it doesn't have the chill plates which are used under each module of the stock Bolt pack. I assume that the chill plates are not in individual sections for each module, and are not included.

Five of the larger modules (which are 10S configuration) plus one of the smaller (which are 8S configuration) would have a nominal voltage of 218 V.

You are correct. We have already separated the original battery pack into 10 smaller packs for EV conversions. You will have to add your own heat sink/ cooling plates.
There are only five 10s and one 8 remaining, as the other packs have been sold.
